Abstract Background Obstructive sleep apnea (OSA) is a unique potent predictor for stroke compared to other predictors. By aiding in the adherence of leukocytes and platelets, soluble P-selectin (sP-selectin) contributes development ischemic stroke. The objective this study was investigate independent impact OSA on platelet activation silent brain infarction. Twenty-four patients 24 controls were studied case–control study, who underwent one-night polysomnography, magnetic resonance imaging evaluation infarctions (SBI), measurement serum levels assessment increased C-reactive protein (CRP) levels. Results Out patients, 5 (20.8%) had mild 8 (33.3%) moderate 11 (45.8%) severe OSA. Serum sP-selectin statistically significantly higher groups ( p < 0.001). Eleven (57.9%) SBI. Fifty percent with elevated 25.0% them SBI 81.8% Patients no normal level. CRP (16.6% 45.8%) than control (4.1% 0%) Conclusion Moderate obstructive associated independently reflecting function, inflammatory marker an risk infarctions.
